Simplify your search from start to finish:

Customize your search by specialty and other criteria, as well as distance from home or work
Save time with the "Find a certain provider" feature - look up a person, group, or facility by name, then see if the provider is in your network

It's easy to use Physician Finder Plus:
Go to the "Members" section of www.humana.com and choose "Enter Member Site".
Choose a provider type under "Provider Search" on the bottom right side of the page. Then you simply:

Select physicians/specialists.
Enter your ZIP code in the box under "Search by Coverage and Network".
Select "Network" from drop-down and choose either UK-HMO-LSA or UK-HMO-RSA, depending upon which plan you have click on "go".
Enter your location (the county that your eligibility is based upon - i.e. live or work). You can search for providers near a certain ZIP code or, if you prefer, within a particular county or mile radius. Don't forget to agree to the Terms of Use on this page, too. Also, please be aware that if you search within a certain mile range, this may return providers that are not in your individual network. REMINDER: With UK-HMO RSA, you can only see providers in your county or an immediately adjoining county or you must come back to UK for your services.
Select a provider type. Click on the type of provider you're looking for. The options vary depending on whether you're looking for a doctor, hospital, urgent care center, or other healthcare provider. For instance, if you're searching for a doctor, you can choose primary care physician or specialist and then choose a particular specialty. Click on "Advanced Search" to pinpoint by gender, certifications, languages spoken, and more.
Get your search results. Click a name to see contact information, a map, and directions. You can also create a PDF of your search results and print your results in an easy-to-read format.
