If you live within a 50 driving mile radius of the UK Chandler Medical Center or in the counties of Anderson, Bourbon, Clark, Fayette, Franklin, Jessamine, Madison, Mercer, Scott or Woodford you should select the UK-HMO Lexington Service Area Directory.
To determine if a Provider is participating, please call UK-HMO Customer Service at (800) 955-8547, or you may search the online provider directory at click here (see below for how to choose a specific category to search).
When you click above, you will need to choose "Enter Member Site". Toward the bottom of the right side of the page you will see a Provider Search box. Please see the description of the categories to search below:
Physician/Specialist: Looking for a participating PCP or Specialist physician or provider (for therapists, chiropractors, podiatrists, home health, durable medical equipment etc, choose Other Facility/Service)
Hospital: Looking for a participating hospital/facility

Search for Participating Providers
Physician Finder Plus is Humana's online "provider look-up" tool - easily guiding you to the latest information about doctors, hospitals, and other healthcare providers in Humana's networks.
Simplify your search from start to finish:
- Find the most recently updated list of in-network providers - quickly and easily
- Get provider phone numbers, addresses, and maps
- Customize your search by specialty and other criteria, as well as distance from home or work
- Save time with the "Find a certain provider" feature - look up a person, group, or facility by name, then see if the provider is in your network
It's easy to use Physician Finder Plus:
Go to the "Members" section of www.humana.com and choose "Enter Member Site".
Choose a provider type under "Provider Search" on the bottom right side of the page. Then you simply:
- Select physicians/specialists.
- Enter your ZIP code in the box under "Search by Coverage and Network".
- Select "Network" from drop-down and choose either UK-HMO-LSA or UK-HMO-RSA, depending upon which plan you have click on "go".
- Enter your location (the county that your eligibility is based upon - i.e. live or work). You can search for providers near a certain ZIP code or, if you prefer, within a particular county or mile radius. Don't forget to agree to the Terms of Use on this page, too. Also, please be aware that if you search within a certain mile range, this may return providers that are not in your individual network. REMINDER: With UK-HMO RSA, you can only see providers in your county or an immediately adjoining county or you must come back to UK for your services.
- Select a provider type. Click on the type of provider you're looking for. The options vary depending on whether you're looking for a doctor, hospital, urgent care center, or other healthcare provider. For instance, if you're searching for a doctor, you can choose primary care physician or specialist and then choose a particular specialty. Click on "Advanced Search" to pinpoint by gender, certifications, languages spoken, and more.
- Get your search results. Click a name to see contact information, a map, and directions. You can also create a PDF of your search results and print your results in an easy-to-read format.
