If you live within a 50 driving mile radius of the UK Chandler Medical Center or in the counties of Anderson, Bourbon, Clark, Fayette, Franklin, Jessamine, Madison, Mercer, Scott or Woodford you should select the UK-HMO Lexington Service Area Directory.
To determine if a Provider is participating, please call UK-HMO Customer Service at (800) 955-8547, or you may search the online provider directory at click here (see below for how to choose a specific category to search).
When you choose the link above, you will need to choose "find a doctor" which is toward the middle of the page. To the right of this page you will see a Provider Search box. Please see the description of the provider type categories to search below:
Physician/Specialist: Looking for a participating PCP or Specialist physician or provider (for therapists, chiropractors, podiatrists, home health, durable medical equipment etc, choose Other Facility/Service)
Hospital: Looking for a participating hospital/facility

Physician Finder Plus is Humana's online "provider look-up" tool - easily guiding you to the latest information about doctors, hospitals, and other healthcare providers in Humana's networks.
Simplify your search from start to finish:
- Find the most recently updated list of in-network providers - quickly and easily
- Get provider phone numbers, addresses, and maps
- Customize your search by specialty and other criteria, as well as distance from home or work
- Save time with the "Find a certain provider" feature - look up a person, group, or facility by name, then see if the provider is in your network
It's easy to use Physician Finder Plus:
Go to www.humana.com or click here and
then choose choose "find a doctor" which you will find in the
middle of the page.
Choose a provider type in the "Provider Search" box on the right side of the page. Then you simply:
- Select the type of provider you are searching for (i.e. physicians/specialists).
- In the "Search by Coverage & Network" box, click on "Employer Group Plan" and enter your ZIP code in the box.
- Select "Network" from drop-down and choose "UK-HMO-LSA" and click on "go".
- Click the box on "I agree to Humana's Terms of Use " of you will not be able to continue.
- Enter the county of the provider you are searching for and choose "go". You can search for providers near a certain ZIP code; with a particular county; within a certain mile radius; or by a certain specialty.
- Select a provider type in the "Choose provider type" drop-down box. The options vary depending on whether you're looking for a doctor, hospital, urgent care center, or other healthcare provider. For instance, if you're searching for a doctor, you can choose primary care physician or specialist and then choose a particular specialty. Click on "Advanced Search" to pinpoint by gender, certifications, languages spoken, and more.
- Get your search results. Click a name to see contact information, a map, and directions. You can also create a PDF of your search results and print your results in an easy-to-read format.
