Eligibility Requirements for Healthcare Organizations
- Must be public or nonprofit private entity
- Must provide direct care services in a health professions shortage area
- Must not discriminate in care to Medicaid and Medicare beneficiaries and
to those unable to pay for health care
- Must match on a dollar for dollar basis loan repayment to the provider
in exchange for the federal loan repayment match
Eligibility Requirements for Health Provider
- Must be Kentucky licensed or certified to practice his/her profession
- Must practice as a primary care provider
- Must sign a contract to practice in an eligible underserved area of
Kentucky for a minimum of two years
- Must have no prior, unfulfilled practice obligations
- Must be a U.S. citizen
- Must be employed by an eligible healthcare organization
- Must not discriminate in providing health care services to Medicare
and Medicaid beneficiaries and those unable to pay for health care
Loan Repayment Amounts
The loan repayment amount is determined by the health care organization and
the health provider and can be up to $35,000 a year for two years (this includes
the federal and health organization matching amounts).

Purpose of Program
To increase the availability of primary health services through
repayment of education loans of health professionals in return for a
minimum two-year practice in a Health Professional Shortage Area.
Application Deadline
For the fiscal year 2008-2009, loan repayment awards will be available
contingent upon continued funding. Applications are accepted from
September 1, 2008 through June 30, 2009, or until funds are exhausted.
