Independent Psychiatric Evaluation $2000

Neuropsychiatric Evaluation with neuropsychological testing $2500
To evaluate brain injury, dementia, toxic exposure, etc. Includes all components of standard exam plus Additional Cognitive Testing Interpretation of lab tests or imaging.

Additional Medical Records Review $350/hr

Consultation with Attorney $350/hr

Testimony-in Court or Deposition $500/hr

Evaluation or records review.

Please make check payable to our billing agent, Kentucky Medical Services Foundation (KMSF) Tax ID #61-0945743, and mail to our office.

The balance of the bill must be paid prior to release of the report. Should additional evaluation or testimony be required at a later date, those expenses will be handled as the need arises.
