Meeting procedures

To: All students

Please keep in mind that you are to have meetings of your advisory committee twice per year. After each meeting, you and your committee need to complete the Committee meeting minutes form. I ask that you do this in a word document and print it out to have the hard-copy signed. Then, bring the signed hard copy to Sherry Howard AND email the (unsigned) completed minutes form to Sherry.Howard@uky.edu, so we can keep a soft copy for the graduate school.

In addition to the minutes form, please print out a copy of the Committee Meeting Assessment Form form for EACH person on your committee to fill in (i.e., score) at the meeting. They should turn them in to you or your advisor, who will give them to me or Sherry after discussing the results with you.

Those of you who are about to take your Qualifying exam should print out a copy of the Physiology Qualifying Exam Assessment form for EACH person on your committee to score at the meeting. Again, they should turn them in to your advisor, who will give them to me or Sherry Howard after discussing the results with you. This is in addition to the grad school form, which will also need to be signed.

Those who are defending your dissertation will similarly use the Physiology Final Exam Assessment form - one for each committee member. Again, give them to me or Sherry after you go over the results.
