Residency Application

Applicant Requirements

  • Graduate of an accredited school or college of pharmacy
  • Licensed or eligible for licensure in the state of Kentucky

Application Process

  • The application can be downloaded here [ms word] or can be sent to you electronically by e-mailing Dr. Divine at hsdivi1@email.uky.edu
  • The application process requires completion of the questionnaire included in the application and submission of a current curriculum vitae, cover letter, official transcript, and three letters of recommendation
  • An on-site interview will be required
  • Application deadline: January 10th
  • Interviews will be conducted beginning the last two weeks of January through the end of February
