Appendix VIII [ms word]: Lab emergency contact information sheet. Post this sign on your lab door with the most current contact information for the PI and lab workers. Please include work and home numbers in space provided and complete the section for the type of work going on in the lab (i.e. radiation work, biologicals, etc.). Please update this sheet every 6 months or when you have a personnel change.
Spill Response Procedure Plan [ms word]:Please print the Spill Response Procedure Plan which describes how to handle a chemical spill in the lab work area. A copy of the plan should be put in the chemical safety manual (teal binder) in each lab and posted in the lab.
EHS training: Lab Safety Training guidelines [ms word]: This information sheet is posted in all the elevators but is available here to be printed out and placed in the lab. Please periodically check for updates and changes in information on this sheet.
College of Pharmacy Emergency Instructions [ms word]: This sheet provides information on emergency procedures that should be followed during an emergency in the College of Pharmacy building.
http://ehs.uky.edu/classes.html: This link provides training procedures for new personnel working in a lab. Three mandatory classes must be taken by all lab personnel. Other classes may also be required depending on the type of work being performed in the lab. Please consult your lab supervisor or the Chemical Safety Officer if you are unsure of classes required in addition to the three mandatory classes.

This site contains the most current chemical inventory for the College of Pharmacy and is updated monthly. To reach this site, go to the "My Network Places" and find the Pharm Public server. Open the "chem folder" where you will find the "Chemical Inventory" folder. There will be a excel file in this location that will contain chemical information.

This site contains all past inspections for the College of Pharmacy labs. To reach this site, go to "My Network Places" and find the Pharm Public server. Open the "COP INSPECT" folder, where you will find all the labs for the College of Pharmacy listed. Open the folder labeled with the lab you would like to see inspection results and select the word file needed.
