Programs for UK Health Plan Members
 

Participation in the PharmacistCARE Diabetes or Cardiovascular (blood pressure and cholesterol) programs is free of charge to all adult UK Health Plan members who have been diagnosed with diabetes or with high blood pressure and/or high cholesterol. In addition to education and counseling, active participants in the diabetes program receive reduced co-payment for test strips.

Choose one of these easy ways to sign up::

PharmacistCARE partners with the UK Health &Wellness program. UK Health & Wellness offers specific programming in the areas of nutrition, exercise, and health awareness and improvement.

The PharmacistCARE programs are conducted by clinical pharmacists of the University of Kentucky College of Pharmacy who are specially trained in diabetes and cardiovascular care. The pharmacists work in collaboration with your physician and other health care providers to ensure you are receiving the best care available. PharmacistCARE medication Therapy Management Services is a comprehensive health management system that includes:

  • An initial health assessment.
  • Comprehensive review of all your medications..
  • A personalized medication treament plan.
  • Education and training to enhance your understanding of medication use.
  • Coordination of the medication therapy management services with your other health care providers to ensure the best outcomes for you.
