Prediabetes Education and Management
 

According to the American Diabetes Association, 54 million people in the U.S. over age 20 have prediabetes with blood glucose levels that are higher than normal, but are not high enough to be classified as diabetes. Most every adult who develops Type 2 diabetes has prediabetes. Research has proven that proactive management of prediabetes can delay or even prevent Type 2 diabetes.

Our PrediabetesCARE program provides you with important health and lifestyle information regarding this medical condition. Our program, is conducted by clinical pharmacists who are certified diabetes educators and a registered dietitian with diabetes expertise. Our pharmacists work in collaboration with your physician and other health care providers to ensure you are receiving the best care available. Group educational sessions are followed by one-on-one counseling on:

  • What is prediabetes?
  • Importance of diet and physical activity for prevention of diabetes
  • Blood glucose monitoring
  • Heart disease, stroke, high cholesterol and high blood pressure and their relationship to prediabetes
  • Medications that may be used in prediabetes
