What is Medication Therapy Management?
 

Medication Therapy Management is a comprehensive group of services provided by pharmacists as part of the Medicare Modernization Act. PharmacistCARE Medication Therapy Management Services has been educating people in the best, most-effective use of their medications since before the law was enacted.

The PharmacistCARE clinical pharmacists are dedicated to helping individuals achieve their best health through the proper use of medications. As a participant in this program, you will receive:

  • An initial health assessment.
  • Comprehensive review of all your medications.
  • An individualized medication treatment plan.
  • Education and training to enhance your understanding of medications.
  • Coordination of the MTM services with your other health care providers to ensure the best outcomes for you.
