Diabetes Education and Management
 

The DiabetesCARE program is conducted by clinical pharmacists of the University of Kentucky College of Pharmacy who are specially trained in diabetes care. The pharmacists work in collaboration with your physician and other health care providers to ensure you are receiving the best care available. Group educational sessions are followed by one-on-one counseling on:

  • Blood glucose monitoring.
  • Prevention and/or reduction of diabetes-related complications.
  • Stress management.
  • much, much more

Participants also receive a periodic newsletter, "Living Healthier with Diabetes."

PhamacistCARE has offices in Lexington at the Kentucky Clinic and Kentucky Clinic North.


What do I have to do to participate?

  1. Complete the required forms.
  2. Attend group educational classes.
  3. Meet with the program pharmacists regularly (every 1-3 months).
