Cardiovascular Education and Management
 

Cardiovascular Disease (CVD) is the leading cause of death for both men and women in the U.S. However, many CVD risk factors can be eliminated or modified. These include high blood pressure, high cholesterol, high blood glucose, cigarette smoking, lack of physical activity, and obesity.

The CardioCARE (blood pressure and cholesterol) program is conducted by clinical pharmacists of the University of Kentucky College of Pharmacy who are specially trained in cardiovascular care. The pharmacists work in collaboration with your physician and other health care providers to ensure you are receiving the best care available. The blood pressure and cholesterol program consists of:

  • Group education classes.
  • Individualized follow-up sessions.
  • Up-to-date materials on cardiovascular care.
  • Periodic cardiovascular disease newsletter.
  • Telephone consulation for those outside of the Fayette County regional area.
  • Telephonic lifestyle case management through the UK Behavioral Health Improvement Program (BeHIP).


What do I have to do to participate?

  1. Complete the required forms.
  2. Attend group educational classes.
  3. Meet with the program pharmacists regularly (every 1-3 months).
