Withdrawal

While enrolled in the College of Nursing, you may need to interrupt your program for various reasons. The University of Kentucky Bulletin covers university withdrawal policy.

You must be cleared by the College and the UK Registrar to maintain the integrity of your academic record.

• Request College clearance: Ask your faculty advisor or Office of Student Services for a College Withdrawal Form. Obtain your faculty advisor's signature and return the form to the appropriate Assistant Dean.

• Request Registrar clearance: Go to the University Registrar's Office, room 10 Funkhouser Building or fax a letter signed and addressed to the UK Registrar, indicating your decision to withdraw. Include your full name, social security number, and remember to sign your name. Fax the letter to (859) 257-7160.
