Body Bequeathal: Forms

You can download the forms required for becoming a body donor, or bequeathing your body to the University of Kentucky Body Bequeathal Program, to your computer by clicking here. To be able to view and/or print these forms, you must have Adobe Acrobat Reader installed on your computer. This is a free program from Adobe. You can download and install Acrobat Reader by clicking here.

Once you complete the printed forms, mail the to the Program Coordinator as indicated in the printed materials.

Should you not have a printer, click here for a list of various methods for contacting the Program Coordinator. Use the most convenient for you. The printed forms will be mailed within three working days. Be certain you supply your full name and mailing address in either your email or voicemail message.
