required for becoming a body donor, or bequeathing your body to the
University of Kentucky Body Bequeathal Program, to your computer
choosing one from the following list:
Once you complete the printed forms, mail them
to the Program Coordinator as indicated on the
Donor Instructions section
of this web site. Should you not have a printer, please
contact the Program
Coordinator. Use the method that is the most convenient for you. The printed forms
will be mailed to you within three working days after you make
contact. Be certain you supply
your full name and mailing address in either your email message or voicemail
