Criminal Background Checks

Some facilities to which students rotate for clinical experiences require criminal background checks as a condition of the rotation. Appalachian Regional Healthcare (ARH) Facilities, for example, currently require a criminal background check done through the Kentucky State Police (KSP). The cost of such checks is currently $10.00 and this cost is the responsibility of the student. It is important, if you are going to be doing an off-site rotation, to check with the course director of that rotation to see if a criminal background check is required by the facility involved. If a background check is required, complete the request form (PDF) and return it to KSP with a check for $10.00 and two self-addressed, stamped envelopes, one addressed to the facility requiring the background check and one addressed to you.
