Resources for Alumni

Transcript Requests
To request a transcript, send the completed and signed Transcript Request Form (PDF) to the address or fax number below. An official transcript is printed on securty paper that cannot be duplicated and includes an embossed College of Medicine seal. Individual transcripts are provided at no charge.

Verification of Medical Education
The Registrar provides verification of medical education for alumni. Only the most basic information (enrollment and graduation dates) will be released without written permission of the student/alumnus.

Medical licensing authorities for each state and facilities that employ physicians each have their own forms and procedures for verification of medical education. Please send the required form, along with a signed Release of Information form (PDF), to the address below. The Registrar will respond to all verification requests within one week of receiving the request, provided that there are no outstanding financial obligations.

Please note: No information or educational records will be released if the alumnus has an outstanding financial obligation to the University of Kentucky College of Medicine.

Other Services
In addition to completing verification forms and filling transcript requests, the Registrar can provide copies of Dean's Letters/MSPE's for all alumni, and certified copies of diplomas for the class of 1981 to the present graduating class. Alumni from earlier classes may send copies of their diplomas for certification, but no copies were retained prior to 1981.

Duplicate Diploma Requests
Alumni may order duplicate diplomas from the main campus Registrar's office for $25.
