Any Kentucky woman 18-89 years old.
Why should I participate?

To help researchers learn more about the factors that affect our health, and in so doing help them improve prevention and treatment of our health conditions.

Is my information confidential?
Yes, absolutely. The Registry has been issued a Certificate of
Confidentiality from the National Institute of Health.
Where is my information stored?
You survey answers are stored in our secure database.
Should I be worried about confidentiality when filling out the online
survey?
No. The survey operates from a secure website.
How often do I need to participate?
We ask that you update your Registry information just once a year.
What if I don't have a computer or an email address?
You can call us at 800-929-2320 and we will send you a paper copy of the
survey along with a return envelope (prepaid postage).
What happens to my paper survey after my information is stored in the
database?
Once we have secured your information in our database we destroy your
paper survey. We retain the paper copy of your consent forms.
How would I be contacted if I were selected for a study?
We would send you a letter describing the study. You can decline
participation in any study and still be a part of the Registry.
Do I have to give personal information like my name, address and date of
birth?
Yes. We do not give that information to anyone. In order to use your
data for statistical purposes we need your date of birth. We need your
name and address in order to contact you in the future.
