
Kentucky Fatality Assessment and Control Evaluation (FACE) Program
Welcome
The Kentucky Fatality Assessment and Control Evaluation (FACE) program is an occupational fatality prevention and surveillance program of the Kentucky Department for Public Health and the University of Kentucky. Kentucky is one of seven FACE states currently funded by the National Institute for Occupational Safety and Health (NIOSH). The Kentucky FACE program has conducted surveillance of all occupational fatalities since 1994.
In addition to surveillance, Kentucky FACE staff members conduct on-site investigations of workplace fatalities, producing investigative reports and other training materials which contain targeted recommendations for workplace injury prevention.

Publications and Reports
Fatality Investigation Reports
These reports are the result of on-site investigations and detail the events surrounding a fatal work injury. Safety recommendations are made based on the hierarchy of hazard control, including elimination or substitution of the hazard, engineering controls, and personal protective equipment (PPE).
Hazard alerts are 1-2 sided documents which address an industry hazard which has been identified as a contributing factor to death in two or more recent Kentucky FACE cases. Made for training purposes and to be displayed in workplaces, these documents provide a brief overview of each fatality and offers safety recommendations for prevention of future similar incidents.

May 2017: The KOSHS Program has released a Preventing
Construction Falls Toolkit, which has a variety of
resources for employers to use to help maintain awareness of
fall hazards on construction sites. This toolkit was created in
recognition of the 2017 National Stand Down Week to Prevent
Falls in Construction.
February 2017: The KOSHS Program has
released an online
training module series which addresses what drivers should
expect during a routine Level 3 Driver Inspection at roadside.
The training can completed at the users pace and offers a
printable record of completion at the end of each module.
Employers can request to have a free portal set up to keep track
of their employees' progress and quiz scores.
September 14, 2016: The FACE Program has
released a new Hazard Alert entitled 'Convenience Store Robberies'. This Hazard
Alert focuses on operational preparedness including general
safety tips to follow in the event of a robbery, and physical
deterrent recommendations on the design and layout of the store
premises.
May 16, 2016: The FACE Program has released
a new workbook entitled 'Preventing Falls in Construction'. This
workbook is intended for classroom training to raise awareness
of the most common causes of fatal falls in the construction
industry.

May 16, 2016: This week marks the beginning of
the National Waste & Recycling Association's Safety
Stand Down Week to prevent heat-related illness in the
waste and recycling industry. Last year alone, at least 30
workers in the US died as a result of heat exposure. Click

April 2016: On the morning of May 5th, 2016, Kentucky FACE Safety specialist De Anna McIntosh conducted a pre-shift Safety Stand-Down Meeting with a construction crew at Messer Construction in Lexington, Kentucky. The talk focused on spreading awareness of fall-related hazards specific to the construction industry. This was done in support of OSHA's National Safety Stand-Down to Prevent Falls in Construction week, spanning May 2-6.
April 2016: Today the Kentucky Transportation
Cabinet begins Work Zone Awareness Week.
Ninety-three workers have died in the line of duty over the
past 50 years on Kentucky roadways. In honor of this important
campaign, we encourage all Kentuckians to be extra mindful of
the people out there working on our roads. We all hold that
responsibility, because we are Kentucky.

April 2016: April is Distracted Driver Awareness Month. Distracted driving is a prevelant safety issue that affects not only civilian drivers but also work-related transportation and travel. January 2016: The National Waste & Recycling Association is hosting it's first National Stand Down Week to prevent injuries and fatalities related to truck backing. The event will take place from January 25-29.

August 2015: The KY FACE fatality report 'Scrap Processor Struck by Wrench and Dies after Makeshift Torque System Fails' was featured in the latest issue of Scrap Magazine. Scrap magazine is a leading trade journal for the scrap processing industry. Check out their latest issue, with our article featured on page 51.

August 2015: The KY FACE fatality report 'Foreman Falls into Running Auger after Stepping on Door Panel with Broken Door Interlocks' was featured in the August issue of EHS Today magazine.
