Student Forms/Syllabi:

Please note there are three different forms for the various types of undergraduate research experiences: a non-credit bearing contract (e.g. for any experience that is voluntary, allows a stipend, or is related to a fellowship or scholarship), a credit course contract that is different from HHS-455 (e.g. to be used for Bio395, KHP-577, etc.), and one specifically for enrollment in the HHS-455 course. *These forms may now be submitted electronically (emailed) to cynthia.byars@uky.edu (click the "Submit" button when completed). The form is to be filled out, signed by the mentor, the CHS Director of Undergraduate Research. A copy of the executed contract will be distributed to both the student and faculty member.
