GCRC Scheduling Procedure

GCRC Patient Registration Form
  1. Study subjects can be scheduled by calling 323-6483 then faxing a GCRC subject registration form (see below) to 323-1118.

  2. All subjects require registration in the hospital's system. We ask that you fax the GCRC subject registration form as soon as you know your subject will be coming to our unit. Be sure to include the study's IRB number and the attending physician name where indicated on the form. http://www.mc.uky.edu/gcrc/documents/OtpRegForm.pdf

  3. We understand there are occasions that you may not have any other information prior to subjects' arrival. In these cases the subject can complete the form in our waiting area prior to their GCRC appointment. We cannot send lab samples to the clinical lab for analysis without the subject's registration in the system. If we have the registration prior to the appointment, the nurses can take the sample to the lab and there is less waiting time for your results.

  4. Admissions to the unit overnight or for extended stays should be scheduled as far in advance as possible, but at the very latest, the Friday of the preceding week. This requirement is to assure appropriate staffing for evenings and weekends, which must be arranged with the hospital nursing staff. If we have no patients scheduled, hospital nurses are released to other areas to cover for the hospital census. Simply check off the proper type of admission on top of the registration form.

  5. We understand that subjects can qualify for studies on short notice and you may wish to schedule walk-ins. We will make every effort to accommodate you by working in to our daily schedule. However, we ask that you call 323-6481 as soon as you know of a subject that needs to be seen without prior scheduling so we can properly prepare for your study. The subjects' waiting time will be decreased if we can prepare materials in advance. Please also be advised that we must honor scheduled appointments prior to walk-ins.

  6. We understand that subjects fail to arrive for appointments, cancel at the last minute, or may fail early screening criteria. In these events, if you have an appointment slot held, we ask that you please call and cancel so that we may accommodate other investigators.

    We hope this procedure is simple for you. Given the increasing number and complexity of GCRC protocols, we need to schedule staff and resources efficiently. We will accommodate your request to the best of our ability and make every effort to ensure the accuracy of sample collection, that quality patient care is rendered, and patient safety is maintained at all times.
