Sports Medicine Fellowship Application

Application Process

We encourage you to apply to our Sports Medicine Fellowship to discover what the University of Kentucky can offer you. The deadline for applications is October 1.

• Universal Primary Care Sports Medicine Fellowship Application form
• Copy of Medical School Dean's letter
• Three letters of recommendation including one letter from Program Director
• Copy of United States Medical Licensing Examination (USMLE) transcript or equivalent
• Copy of Medical school transcript

Interviews

• Interviews are scheduled by invitation only from October through through mid-December.
• Interviews begin at 10:15 a.m. and end about 2:30 p.m.
