The surgical fellow program is now AUPO FCC compliant. All applicants must register through the ophthalmology fellowship match under the National Residency Matching Program. To register for the fellowship match, contact www.sfmatch.org. In addition to the requirements by the fellowship match program, we would like all applicants to complete a UK ophthalmology application form, submit an updated curriculum vitae, an official transcript record from their medical school, three letters of recommendation, and a personal statement indicating what your goals are in relationship to this fellowship as well as long-term goals.
