Dear Applicant:
Thank you for your interest in our Vitreo-Retinal Ophthalmic Fellowship beginning July 7 of each year. These fellowship programs are a joint effort of the University of Kentucky and the private practice of Retina Associates of Kentucky. Previously both programs trained fellows in retinal and vitreous disease. One two-year surgical and one one-year medical fellowship will be offered. Requirements at the two locations may differ slightly, but the objectives and focus are identical.
The application must be completed in its entirety and returned by September 01 to be considered for an interview in the fall (specific dates to be announced at a later time). In addition to the requirements outlined on the application form, we are asking you to provide us with an updated Curriculum Vitae, three (3) letters of recommendation, one of which must be from your Residency Chairman or Program Director, and a personal statement indicating what your goals are in relationship to this fellowship as well as long-term goals, and your Ophthalmology Fellowship Matching Program number. If you are a graduate of a foreign school, please attach a copy of your ECFMG.
