Oral Diagnosis/Oral Medicine/Oral Radiology Record Keeping in Dental Practice
Registration and Learning Format
This course can only be taken by correspondence. This predominately self-instructional course is designed to re-familiarize dental health professionals with the fundamentals of effective record keeping in the office. It is estimated that the participants will spend approximately 3-5 hours in the completion of the course.

Course Overview
The course will consist of reading assigned articles, viewing a Powerpoint presentation, a review of example chart entries, and a written examination. A score of at least 80% is required to pass the course.
Course Objectives
Following completion of the course, the dental health professional should understand:
· The purposes of a clinical chart
· What information should and should not be included in a patient’s chart
· What format should be used to record information in the chart
· How chart entry errors should be corrected
· Who has ownership of the chart and the release of records
· How to document adverse events or patient behavior
· Who should make chart entries and who should sign them
· What is the need for a written treatment plan
· How long patient records should be maintained
