REGISTRATION INFORMATION. Participants are requested to pre-register at least four weeks in advance.  Applications for registrations are processed in the order received and must be accompanied by the registration fee.  Your registration will be acknowledged in writing if time permits.  You can register by phone, fax, e-mail or by mail.  Early registration is recommended to ensure that a place is available and to allow for confirmation of your registration. 

Payment. Mail registration form with a check made payable to UK College of Dentistry CE to:

University of Kentucky, College of Dentistry, Continuing Education, 1117 South Limestone Street, Lexington, KY  40503-1057.  Visa or MasterCard are also accepted.

PARKING ON CAMPUS. On weekends, free parking is available in the Kentucky Clinic parking structure, across the street from the UK Chandler Medical Center and the UK College of Dentistry.  On week days, contact the Office of Continuing Dental Education regarding parking as it varies with each course.

A map showing available parking facilities within walking distance of the College of Dentistry will be mailed along with your confirmation letter.

HOTEL INFORMATION. Participants are responsible for making their own reservations.  Along with your confirmation letter you will receive a list of area hotels.

COURSE CREDIT. The University of Kentucky College of Dentistry is an ADA CERP recognized provider.  All programs are approved for Fellowship and Mastership credit by the Academy of General Dentistry.  All programs sponsored by the University of Kentucky for dentist and dental hygienists are approved by the Kentucky Board of Dentistry for credit of one point per hour to meet relicensure requirements.

REFUND AND CANCELLATION POLICY. A refund, less a $25 processing fee, will be issued for all cancellations received/postmarked at least two weeks prior to the course date.  A 50% refund less a $25 processing fee will be given for cancellations received one week prior to the course date, with no refund thereafter.  Cancellations received the day of the course are not eligible for a refund.  Registratants may choose to receive a credit toward another course. 

COURSE CANCELLATIONS. UK College of Dentistry reserves the right to cancel any course when circumstances are warranted.  In such a case, a full refund will be issued.  Also, UK College of Dentistry will not be held responsible for travel arrangements made by the participant.
