                  Admission Requirements

Admission requirements for the CLM Program include an Associate Degree in a health care discipline and one year's post-degree work in a health care setting. An application package (download - save to your desktop first - or available through the Office of Student Affairs, College of Health Sciences), official school transcripts, and three letters of recommendation must be submitted to the Office of Student Affairs. Please download - save to your desktop first - and complete the appropriate application materials (Form A-Application; Form B-Reference Form (print 3); Form C-Courses Planned/In Progress; Form D- USP; and Form E-Community Service/Work Experience). Once completed, ALL forms are to be mailed to the Office of Student Affairs and Admissions in ONE PACKET.

Applications will be reviewed and admission to the program is on a rolling basis. However, at the time of acceptance to the CLM Program, the student must already have been accepted to the University of Kentucky. For spring semester acceptances, university applications must by submitted by December 1 and for fall semester acceptances, they must be submitted by August 1.

To receive a Baccalaureate Degree from the College of Health Sciences in Clinical Leadership and Management, students are required to complete a minimum of 120 course credits which includes the 39-credit core curriculum, completion of the University Studies Program, and UK's writing and logic graduation requirements. It is advisable that prospective students discuss transfer credits from their associate degree program with Ms. Marilyn Underwood, Academic Advisor, College of Health Sciences Office of Student Affairs. The College's Office of Student Affairs will also assist CLM Program faculty in monitoring and tracking their students' completion of the program, and USP and UK graduation requirements.
