I'm the Clinic Manager at the Bluegrass Care Clinic. My job is to make sure you are happy with the services and treatment you receive.

I want to hear from you. Especially, if you feel we have let you down in any way. Please help me, help you by calling or emailing me. Your feedback will not only help improve the services you receive but those that other patients receive as well.

All complaints are confidential and can remain anonymous if desired!

Details I will need to know:

(1) Nature of the Complaint
(2) Details about the Complaint
(3) Any dates/times associated with the
incident that sparked the Complaint
