| Frequently Asked Questions (FAQs) About Care Coordination:1) What is the Care Coordination program? The Kentucky Care Coordination Program (KHCCP) facilitates the provision of quality care and services by helping to eliminate barriers to care. These barriers may be financial, emotional, social, and/or medical. The ultimate goal of KHCCP is to help persons living with HIV/AIDS become self-sufficient. 2) Who funds the Care Coordination program? The Federal Ryan White Program funds the state of 3) Who is eligible for Care Coordination? Clients that are at or below 300% of the federal poverty level are eligible for care coordination. However, any client may receive support from a care coordinator if he/she needs it. 4) Do I have to have a Care Coordinator? If you are receiving financial assistance from the Kentucky AIDS Drug Assistance Program (KADAP), the Kentucky Health Insurance Coverage Program (KHICP), or AVOL, Inc, you are required to have a care coordinator. The care coordinator verifies household income for these state programs. 5) How often will I see my Care Coordinator? That depends based on your need. Your care coordinator might see you every time you come to your medical appointments, or he/she might only see you once a year. At a minimum, the care coordinator is required to see you in person at least once a year, and speak with you at least one other time. The care coordinators are available when you need them…all you have to do is ask! 6) Will the Care Coordinator come to my house? With your permission, your care coordinator might provide a home visit. This is more likely to occur if you are not able to meet with your care coordinator in the clinic. 7) How do I know who my Care Coordinator is? Your care coordinator is assigned to you at the time of your intake appointment. If you are uncertain who your care coordinator is, you should call the social services supervisor at 323-6303. 8) Sometimes I talk to a Care Coordinator that is not assigned to me…why? The care coordinators cover different doctors’ clinics. In addition, they work as a team and sometimes cover for each other. You may receive services from any member of the care coordination team. 9) Can I switch Care Coordinators if I prefer someone else? We try and keep case loads equitable among the staff. But if you feel that you “click” with one particular care coordinator, please notify the social services supervisor at 323-6303. 10) What can my Care Coordinator help me with? Housing, food, medicine, medical supplies, transportation, substance abuse treatment, mental health services, intimate partner violence, employment, social security and disability insurance, health insurance, social support, and much, much more! 11) What are my responsibilities in care coordination? Your job is to keep us informed of any changes to your housing, employment, income, benefits and/or insurance. In addition, we ask that you work with us to develop and obtain short and long-term goals that help you reach self-sufficiency. 12) What should I bring to my meeting with my care coordinator? • Proof of income such as: two most recent pay-stubs, most recent W-2, check stub from SSI/SSDI, or a food stamp award letter • Proof of Kentucky residency such as: driver’s license, copy of signed lease agreement, current utility bill, or statement from a person providing room and board • Proof of health insurance if applicable 13) How can I reach my care coordinator?
