This includes HIV and related issues. The Bluegrass Care Clinic's mission is to
provide a continuum of high quality, state-of-the-art,
multi-disciplinary HIV primary care in a compassionate, culturally sensitive manner.
Our goal
is to achieve 100% access to HIV primary
care with 0% socioeconomic disparity in health outcomes. You are the key.You, your physician, Patient Services Coordinator, and Care Coordinator will identify both your strengths and challenges to develop a comprehensive treatment plan. This treatment plan will address immediate physical needs as well as other health issues, including emotional well-being, nutrition, medications, dental care, and pain management. Located on S.
