Introduction
All students enrolled in one of the
UK health professions programs will be required to complete
one or more immersive community-based clinical experiences at a site
external to the Chandler Medical Center. These
experiences range in duration but must be at least four weeks in
length. The AHEC program is committed to supporting these activities
for UK students. AHEC support includes reimbursement for rent
and mileage during the rotation. The details of this support and
more are included in the AHEC Student Manual. You can read or
download the entire manual below. The remainder of this page
provides access to required forms and additional information.

No
later than four to five weeks before beginning your rotation, you
must complete an AHEC Fact Sheet (provided by your college’s or
department’s AHEC Coordinator or obtainable from the AHEC Central
Office or website). The AHEC Fact Sheet provides the program with
reliable data on student and resident rotations as well as serves as
a mechanism for communicating among AHECs to activate specific
services for students and residents as they initiate their
rotations. If for any reason a rotation is canceled or a site
changed, please contact the AHEC Central Office. Every UK
student/resident who takes an off-site rotation is expected to
complete the Fact Sheet.

Rotation Assessment Form
The
Off-Site Rotation Assessment Form has been developed for the AHEC
Program with multiple purposes in mind. Primarily the AHEC Program
wants to monitor certain aspects of the program and your assessment
and observations are pivotal to the accurate evaluation of how well
the program is doing. Every student and resident is expected to
complete the assessment form upon completion of his or her rotation within one week following the off-site rotation. If you have
multiple rotations, please fill out an assessment form for each
rotation. This is important because the change in rotation setting
and preceptor may make a
significant difference in the kind of experience you have.
The assessment form is “user friendly” and can be completed
within a reasonable length of time. Your assessment is important to
our program and enables us to assess how well we are accomplishing
some of our objectives. At the same time, your assessment will help
you reflect on your experiences and evaluate some of your own
reactions, attitudes and experiences. The information provided will
be considered confidential.
As you look over the assessment form, you can see that you will be
evaluating three specific aspects of the off-site rotation: the
administrative aspects of the AHEC Program, attitudes and
understandings regarding your off-site experience and the
educational experience of the off-site rotation. The evaluation
scales are self-explanatory. Once again, please complete the
assessment no later than one week following your rotation at the
same time you settle your account.

Rent Receipt Form
The
AHEC Rent Receipt Form (Appendix IV) must be completed by you and
signed by the lessee and returned to the AHEC office at the
completion of your rotation. This form must be turned in to receive
reimbursement for mileage.
