Physician/Specialist:  Looking for a participating PCP or Specialist physician or provider (for therapists, chiropractors, podiatrists, home health, durable medical equipment etc, choose Other Facility/Service)

Hospital:  Looking for a participating hospital/facility

Pharmacy:  For Customer Service Call Center (available 24 hours a day, 365 days a year) at (877) 242-1864, hearing impaired (800) 899-2114, or to visit the Express Scripts Web site click here.

Dentist - Looking for participating oral surgeons

Urgent Treatment Center - Looking for participating Urgent Treatment Centers
