UK-HMO members now have on-line access to MyHumana, which allows you access to the health care information you need 24 hours a day, seven days a week at your convenience.  From this site you can access the following:

• View claims status, referrals, and Prior Plan Approvals

• Access important health care information

• Search for participating providers and facilities

• Access your Certificate of Coverage (COC)

• View plan benefit details

UK-HMO MEMBERS WILL NOT BE ABLE TO ADD OR TERMINATE  DEPENDENTS FROM THIS SITE.  YOU WILL NEED TO CONTACT EMPLOYEE BENEFITS TO COMPLETE THIS PROCESS.  

You will need to create a sign-on before you can view any information.  Due to HIPAA regulations, subscribers may only view information pertaining to the subscriber or their children under the age of 18.  All other information must remain confidential.  If you require access to a spouse or a child over 18 years of age, you would need to contact the member services number on the back of your ID card and ask them to send you a Designation of Personal Representative Form to have that family member sign and return.  Once this is received, access will be granted to the appropriate individuals.
