UK North Fork Valley Community Health Center

Community health centers are delivery sites for medically underserved populations to receive comprehensive primary and preventive health care services to all persons without regard to an individual’s ability to pay.

UK North Fork Valley Community Health Center is a joint venture between the University of Kentucky College of Medicine and the community health board. It is the first community health center in Kentucky to be affiliated with a university and family medicine residency training program and is located in Hazard, KY, at the UK Center for Excellence in Rural Health-Hazard.

UK North Fork Valley Community Health Center provides many types of service and offers a sliding fee scale to help reduce the financial burden that health care costs can create. All patients are treated regardless of their income or ability to pay.

Hazard Medical Clinic - UK North Fork Valley Community Health Center is pleased to offer integrated and comprehensive access to quality Primary Care health services. Our Primary Medical care services are provided in collaboration with our Dental and Behavioral health services at our clinic locations by qualified Family Practice care providers offering the full range of medical health services from newborn well child care, maternal prenatal care, on through to comprehensive geriatric care.

Dental Clinic - UK North Fork Valley Community Health Center is pleased to offer integrated and comprehensive access to quality oral health services. Our services are provided in collaboration with UK clinic providers, and encompass a full range of dental services across the life span, including mobile dental outreach via a Ronald McDonald Care Mobile at area schools and childcare centers.

Psychological Services - UK North Fork Valley Community Health Center is pleased to offer integrated and comprehensive access to quality behavioral health services. Our services are provided in collaboration with UK clinic providers, and encompass a full range of psychological services across the life span.

Pharmacy Services - The UK North Fork Valley Community Health Center Pharmacy is dedicated to providing quality pharmacy services to the people of Eastern Kentucky.

Laboratory Services - The UK North Fork Valley Community Health Center Laboratory provides our patients with quality patient testing services that are convenient, efficient, timely, and reliable.

Women’s Health Services - UK North Fork Valley Community Health Center is proud to offer comprehensive women’s health programming as part of our multi-disciplinary team of professionals.
