The UK North Fork Valley Community Health Center is pleased to announce that we have expanded our service area to include the HomePlace Clinic located in Ary, Kentucky.

To continue to provide quality medical services to the Ary, Kentucky residents, Dr. David Moore, M.D. began seeing patients at the HomePlace Clinic on August 1, 2007.

All patients are treated regardless of their income or ability to pay. We accept all insurances.

Our Clinic has a sliding fee scale to help reduce the financial burden healthcare costs can create.

We provide the following services.

• Preventive Medicine
• Complete Physicals
• Pediatric Care - Acute and Well Child
• Geriatric Care
• Sports Physicals
• Dietary Services
• Laboratory Services
• Immunizations - Child and Adult
• Acute Illness, Injury, and Minor Emergency Services
• Pre-employment, DOT, and School Exams
• Family Planning

The clinic is open Monday, Tuesday, Thursday, and Friday from 8 a.m. to 6:00 p.m.
