Application Information
**NOTE** The PGY1 applications will be available to download on Thursday, December 10th, 2009.
The following items will be required for application submission:
- Residency program application
- Letter of interest
- Curriculum vitae
- Three letters of recommendation with accompanying evaluation form
- One from a pharmacy employer
- One from a clerkship (advanced pharmacy practice experience) preceptor
- One from another preceptor or pharmacy faculty member
- Transcript from all pharmacy training completed*
*May be through Summer Semester 2009, but a current transcript including Fall Semester will be required at interview.
