Step 1:
Apply to the Graduate School.
Submit the following to the University of Kentucky

Graduate School Application for Admission and Application
Fee.
Complete the application and pay application fee online.
Contact the Graduate School at (859) 257-4613 with questions
regarding on-line application or fee payment process.
One official transcript from EACH college/university
attended.
These must be mailed directly to the Graduate School by the
institutions. Official transcripts must be enclosed in a
sealed envelope from the issuing university and marked as an
official transcript. Those marked as issued to student, or
which are hand-delivered, are not acceptable. Send
transcripts to: UK Graduate Admissions, 101 Gillis Bldg.,
Lexington, KY 40506–0033.
Official GRE Scores (Verbal, Quantitative, and
Analytical Writing).
These must be sent directly from the Educational Testing
Service (ETS) to the Graduate School. Hand-delivered copies
are not acceptable. Use University of Kentucky Institution
Code: 1837. Code for nursing: 0610.
Official TOEFL Scores (international students only).
UK requires a score of at least 550 on the Test of English
as a Foreign Language (TOEFL) or 213 on the computer-based
test for all applicants whose native language is not
English. TOEFL scores must be sent directly to the
University of Kentucky Graduate School from Educational
Testing Service [ETS]. Photocopies are not acceptable.
Goal Statement (no form). Your goal statement should be
1-3 typed pages, double-spaced. (See instructions on page 2 of
application form.)
College of Nursing
Application for Scholarship/Service
Award. NOTE: Preference for financial
assistance will be given to applications completed by the
above application deadlines.
Reference Forms (print three copies). References should be chosen from among the
following: current employer, nursing faculty member, immediate
past employer, nursing or health professional. (A Placement
Service folder will be acceptable as one of the three
necessary references.) Of the three references, two should be
from nurses.
