Request two official transcripts from all
colleges or universities attended, to be mailed directly
to the Graduate School. (Hand-delivered or "student"
copies are not acceptable.)
Request official GRE scores from the
Educational Testing Service (ETS) to be mailed directly
to the Graduate School.
NOTE: If scores are no longer available from ETS, request a
letter from the school from which you received your M.S.N.,
verifying your official scores and the date you took the GRE.
This letter must be mailed directly from your
former school to the Graduate School.
Goal Statement (no form). Your goal statement should be
1-3 typed pages, double-spaced. (See instructions on back of
application form.)
The College of Nursing
Application for Scholarship/Service
Award. NOTE: Preference for financial
assistance will be given to applications completed by the
above application deadlines.
Reference Forms. (Print three copies.) References should be chosen from among the
following: current employer, nursing faculty member, immediate
past employer, nursing or health professional. (A Placement
Service folder will be acceptable as one of the three
necessary references.) Of the three references, two should be
from nurses.
