Applying to the D.N.P. Program
Send the following to the University of Kentucky, College
of Nursing, Office of Student Services, 309 College of
Nursing Bldg., Lexington, KY 40536-0232:

Official transcript from
each college/university attended
These must be mailed directly to the College of Nursing by the
institutions. Official transcripts must be enclosed in a sealed
envelope from the issuing university and marked as an official
transcript. Those marked as issued to student, or which are
hand-delivered, are not acceptable.

Photocopy of current R.N. license
-
Official Graduate Record Examination (GRE) scores (Verbal,
Quantitative, and Analytical Writing)
These must be mailed directly from the Educational Testing Service to the
College of Nursing. Hand-delivered copies are not acceptable.

nursing code 0610.)

Three references (forms provided-pdf format)
These can be sent by a current employer, nursing faculty
member, immediate past employer, or nursing or health
professional. Of the three references, two should be from
nurses. You will need a set of reference
forms for each of your references.

Written Goal Statement

Portfolio
of past work (REQUIREMENT WAIVED FOR
FALL 2008 APPLICANTS)
Enclose examples of work you have done
that best reflect your experiences and past education. The
portfolio will be used to help tailor your learning experiences in
the D.N.P. program, so please include samples that best represent
you. This should serve as professional representation of your
background and work. The faculty prefer to see actual
samples of your achievements. Do not send originals as
portfolios are not returned. Portfolios should be assembled in a folder or binder;
include section dividers. Include a sample of each of the following:
- Educational experiences
- Professional activities
- Clinical experiences
- Community activities
- Scholarly endeavors

Application for
Scholarship/Service Awards for Graduate Students
The
earlier the entire application process is completed, the greater
the opportunity will be for support. Preference in
financial assistance will be given to those applicants who have
completed the application process by Jan. 15.

One-time, non-refundable application fee of $100.
Make
your check payable to the UK College of Nursing.

International Students
International students have additional requirements. Please
contact the College's Student Services Office for more information.
Application Deadlines
The entire College of Nursing application process should be
completed by Feb. 15 for the fall semester. Each D.N.P.
class usually begins in the fall. Applications received after Feb.
15 will be considered on a space-available basis.
