Student Forms/Syllabi:

Please note there are three different forms for the various types of undergraduate research experiences: a non-credit bearing contract (e.g. for any experience that is voluntary, allows a stipend, or is related to a fellowship/scholarship), a non-HSE595 credit course contract (e.g. for Bio395), and one specifically for enrollment in the HSE-595 course. The form is to be filled out, then signed by the mentor and student. Bring a copy to the Office of Research in Wethington, Room 105. The Director of Undergraduate Research in CHS will then sign & an executed copy will be emailed to both the student and faculty member.
