Application, Admissions and Tuition
The application deadline is July 30th of the year prior to the planned admission date. There are two applications required: the College of Dentistry and the University Graduate School. Applicants to the Periodontology program must also complete a Graduate School Application. Both applications must be submitted on-line and are available to download by selecting the blue underlined areas

We will, of course, need an transcript from your dental school. International applicants are encouraged to apply. Well-qualified international applicants should contact the program director (see below) for specific information regarding prerequisites, which may differ slightly from those listed below. To complete your College of Dentistry Graduate Periodontology application file, in addition to the on-line application, US applicants should forward the following documents to: OHP/Periodontics Division, College of Dentistry, University of Kentucky, Lexington, KY 40536-0297:
- 4 letters of recommendation
- National Board Scores
- Pre-dental
- Dental Transcripts

In order for the application to be considered the Graduate School must have received the application fee ($50 for domestic and $65 for international students). The applicant should not send official transcripts to the Graduate School at this point. Only when admitted to the periodontology program should the applicant provide the Graduate School with official transcripts, and NBDE 1 or NBDE 2 or GRE scores.
