Undergraduate Application:
The following documents are required for consideration of admission into the CRS undergraduate program:
- Recommendations (3): Recommendation forms
(PDF) may be placed in a sealed envelope with recommender's signature across the seal of the envelope to be mailed with the application. Alternately, the recommendation form may be mailed or faxed directly to the CLS program.
- Official transcripts from all higher education coursework attempted
Use the on-line forms above. Application packets for the CLS undergraduate program are also available through the
Office of Student Affairs of the College of Health Sciences.
Application Deadline:
The deadline for application to the program is
April 1st.

Admission Criteria:
Admission to the CLS program is based upon:
- Minimum cumulative grade-point average of 2.75 on a scale of 0-4 for all courses that are taken at institutions of higher education
- Interviews with members of the CLS faculty
- Letters of recommendations
